I want to link my Cuore Rossonero Card code to my profile, but the error that the card is already associated to another profile appears: how can I solve that?

If you can't access the old account, write to mymilan@acmilan.com, specifying the e-mail corresponding to the MyMilan profile for which you are writing and specifying the card code.
